Caroline has years of experience providing service coordination for individuals and families.  She offers a range of services and supports for families looking to generate comprehensive and sustainable plans or access services that meet their loved ones needs.

Rachel Reesor, M.S.W.L.C.S.W., R.S.W

Rachel Reesor graduated from Virginia Commonwealth University with her advanced standing Master’s of Social Work. She then completed three years of supervised hours and board examination to earn her LCSW, which in the USA grants diagnostic rights for mental health disorders. She has been working as a registered social worker for 6 years, and providing therapeutic social work services for 10 years. During her studies she received training in adult and children’s mental health and diagnostics, play, sand and art therapy, individual and family systems, groups dynamics and facilitation, crisis intervention, mindfulness, Dialectical Behavior Therapy, and trauma informed care. 
 
During her career Rachel has worked in a number of settings supporting children, families, and adults of all ages using various client centered approaches to build on strengths. Her work is trauma informed and strives to be holistic in nature. Her goal is to help people realize their true potential by giving them tools to overcome adversity and build upon their strengths.

  5. Provide 4 weeks of service coordination by collaborating with service providers to support implementation of the service plan and actively advocate where necessary to ensure that it is sustainable
